  I visited this office because it was closer to my house. I asked all of the "normal" questions of a parent who is bringing their child in for immunizations. I even spoke to them via phone immediately before the appointment and said "my son needs proof of vaccinations (forms) signed for school". I was NEVER told of the fee. And, I had visited this office once before and recalled their card machine was down, but didn't think to ask again because no business' credit card machine is down for months. Fatah (not calling him doctor) gave my son his vaccines. He taunted a 4 year old for crying after receiving 3 shots.. Also, I was not told that the 4th vaccine for Polio was not available (conveniently) until after my son had received 3 vaccines. Fatah had no empathy. My son had received numerous shots/vaccines but from caring professionals. So, now I have a 4 year old who has just been picked at and given 3 injections in his ARMS. He is crying hysterically. Now that I am trying to get the forms required for school, I was told that I had to pay upwards of $18 cash to get the forms... A cash payment was required because their card machine was conveniently "still down". So, now I am hysterical too because they will not give me the required forms without payment. I had 2 conversations with this office and was NEVER told of the fee. I called them en route and was NEVER told the card machine was down. My son is still crying uncontrollably. My only options were to drive to an ATM with a crying child.. or hunt in my car for loose change until I came up with the $18. To add to matters, bad doctor, non-compassionate office staff, incomplete shot record, missing shots, crying child, etc... they never gave me a receipt. Imagine how many other parents they are doing the same way. AVOID THIS OFFICE! I will also file a complaint with my insurance company. $18 for forms in cash, without a receipt. They are doing more than cheating parents and mistreating kids, they are avoiding paying taxes.
